The scholarship program is intended to serve children who, without this fund, would be unable to participate in EYSA sports.  It is intended to be a resource after other sources of funding have been utilized.

 

ONLY PARENTS OR LEGAL GUARDIANS MAY APPLY FOR A SCHOLARSHIP.

 

This scholarship program is dependent upon donations and fund-raising and, therefore, a limited amount of funding is available.  No guarantee of assistance is implied in this application.  Scholarships are provided on a first come, first served basis to those who meet the eligibility requirements and who have submitted a complete application during the open enrollment period.  Incomplete applications and applications received prior to open enrollment will not be accepted. 

 

Approved scholarships will receive financial assistance for one individual for one sport.  Individuals must reapply for each sport they are requesting assistance.

 

REQUIREMENTS FOR ELIGIBILITY

 

  • Athlete must attend or reside in the Eatonville School District boundaries.
  • Commitment to attend a minimum of 80% of scheduled practices and games.
  • Not be currently served by an existing scholarship or fee waiver program.
  • Participation by an adult family member in volunteer activities during the sports season to help offset the cost of this scholarship (i.e. helping with fund-raising activities, donating baked good for bake sales, volunteering to coach a game if the coach is unavailable, etc.)  Scholarship recipients will be required to provide 1 hour of volunteer work to EYSA for every $15 they receive in scholarships.  All hours must be completed by the last game of the season for which you received a scholarship.  Failure to complete your volunteer hours will make you ineligible to receive future scholarships.

 

Consideration will be given to eligible youth meeting one or more of the criteria below.  The more information you can provide us, the better able we will be to determine the degree of hardship.  A written request is required.

 

  • Receiving state assistance (i.e. food stamps, TANF, SSI, foster care, etc.).  Must provide documentation of participation in these programs.
  • Written recommendation by school representatives, social workers, youth community center workers or other social services representatives.
  • Documentation demonstrating an immediate financial hardship.

 

CONFIDENTIALITY:  EYSA will use the information on the application only to decide if your child qualifies to receive a scholarship. Confidentiality will be maintained at all times.
