All coaches, assistant coaches, and game officials must review the CDC Heads Up Fact Sheet For Youth Sport Coaches (found HERE).
Once you have reviewed the fact sheet, print out the quiz questions HERE. Using the quiz, enter your "true" and "false" answers to each question below. If you score less than 100%, you will receive notification to retake the test.
This quiz must be completed prior to any coaching/referee activities.
