INSTRUCTIONS: ENROLLING AS A COACH, ASSISTANT COACH, OR TEAM
PARENT IN A PROGRAM

1. Navigate to www.eysaeatonville.com.
2. Click “Sign In” on the top right hand corner of the
page.
3. Login to your TeamSideline account.
4. Click on “Place Order”.
5. Select the enrollee that you would like to enroll as a
coach.
6. Select “Volunteer” in the “Enrollment Type” box.
7. Select the program that you would like to enroll in via
the “Program” box.
8. Select the offering that you would like to enroll in via
the “Offering” box.
9. Select “Coach”, “Assistant Coach”, or “Team Parent” under
the “Enrollment Selection” menu.
10. Click “Add
Enrollment” (a dialog box will appear that indicates that your enrollment has
been added to your cart).
a. To close this dialog box, click the “x” in the top right
hand corner.
b. The cost of enrolling as a coach is displayed in the
“Your Cart” section towards the bottom of the page.
NOTE: Being a coach, assistant coach, or team parent does
not cost anything, but you must complete the checkout process to complete your
enrollment.
11. Once you are
finished enrolling, click “Proceed to Checkout”.
12. Provide the information required to have a
criminal background check conducted.
13. Review the “EYSA
CONCUSSION COMPLIANCE” disclosure statement.
a. Click the box to indicate your agreement with the terms
therein.
14. Review the “EYSA
DRUGS, ALCOHOL & TOBACCO COMPLIANCE POLICY”.
a. Click the box to indicate your agreement with the terms
therein.
15. Review the “EYSA
WAIVER/RELEASE”.
a. Click the box to indicate your agreement with the terms
therein.
16. Click
“Done”.
17. Complete the required billing information
(NOTE: the total cost for enrolling as a
coach, assistant coach, or team parent is $0).
18. Click “Place
Order”.
a. A yellow dialog box will appear and confirm your
enrollment.
Congratulations! You
have successfully enrolled as a coach, assistant coach, or team parent in an
EYSA program!
