INSTRUCTIONS: ENROLLING A PLAYER IN A PROGRAM

1. Navigate to www.eysaeatonville.com.
2. Click “Sign In” on the top right hand corner of the page.
3. Login to your TeamSideline account.
4. Click on “Place Order”.
5. Select the child that you would like enrolled in a program in the “Enrollee” box.
6. Select “Player” in the “Enrollment Type” box.
7. Select the program that you would like to enroll your child in under the “Offering” box.
8. Click “Add Enrollment” (a dialog box will appear that indicates that your enrollment has been added to your cart).
a. To close this dialog box, click the “x” in the top right hand corner.
b. The cost of enrolling your player is displayed in the “Your Cart” section towards the bottom of the page.
9. If you have additional children to enroll, repeat steps 5-8.
a. After adding each enrollment, the total cost of enrolling your children will accumulate at in the “Your Cart” section of the page and display a total.
10. Once you are finished enrolling your player into their program, click “Proceed to Checkout”.
11. Note the name of the player on the top of the next screen. Fill out the required information for this player only.
a. Select “Not at this Time” under the “Volunteer Position” box.
12. Review the “EYSA CONCUSSION COMPLIANCE” disclosure statement.
a. Click the box to indicate your agreement with the terms therein.
13. Review the “EYSA DRUGS, ALCOHOL & TOBACCO COMPLIANCE POLICY”.
a. Click the box to indicate your agreement with the terms therein.
14. Review the “EYSA WAIVER/RELEASE”.
a. Click the box to indicate your agreement with the terms therein.
15. Click “Next”.
16. Repeat steps 11-15 for each child enrolling in an EYSA sports program.
17. Click “Done”.
18. Complete the required billing information.
a. If you would like to pay using an existing PayPal account, login to PayPal and follow the instructions.
b. If you do not have a PayPal account, click “Pay with Debit or Credit Card”.
19. Enter payment information on the “PayPal Guest Checkout” page and follow instructions to complete/confirm your payment.
Congratulations! You have successfully enrolled your player in an EYSA program!
